Upon completion of this course, participants will be able to:

• Apply a Systematic Approach: Utilize a structured ABCDE (Airway, Breathing, Circulation, Disability, Exposure) framework for the initial assessment and stabilization of any ED patient.

• Generate and Refine Differential Diagnoses: Develop broad, logical differentials for common complaints and use history, physical exam, and point-of-care testing to narrow them down.

• Interpret Key Diagnostic Findings: Rapidly interpret ECGs, X-rays, CT scans, and lab results relevant to emergent conditions.

• Manage Common High-Acuity Presentations: Formulate evidence-based initial management plans for critical conditions like sepsis, ACS, stroke, and trauma.

• Recognize “Can’t-Miss” Diagnoses: Identify subtle presentations and red flags for life-threatening conditions that must not be overlooked.

• Enhance Communication and Handover: Effectively communicate assessments and plans within the team and perform structured handovers (e.g., SBAR).

Day 1: The ED Approach & Cardiorespiratory Crises

• Morning:

  • Module 1: The ED Mindset: Introduction to the course, triage systems, and the ABCDE approach to any patient.

  • Module 2: Chest Pain & Acute Coronary Syndromes (ACS): Risk stratification, ECG interpretation, and initial management of STEMI/NSTEMI/Unstable Angina.

• Afternoon:

  • Module 3: Dyspnea & Respiratory Failure: Rapid assessment of asthma, COPD, pulmonary embolism, pneumothorax, and heart failure.

  • Case-Based Session 1: The Crashing Patient. Simulations on managing ACS, massive PE, and tension pneumothorax.

  • Day 1 Recap: Stabilizing the airway and circulation first.

Day 2: Neurological and Abdominal Emergencies

• Morning:

  • Module 4: Altered Mental Status & Stroke: Differentiating between stroke, seizure, metabolic, toxic, and infectious causes. NIH Stroke Scale and tPA/Thrombectomy criteria.

  • Module 5: Headache & Syncope: Identifying red flags for subarachnoid hemorrhage, meningitis, and cardiac syncope.

• Afternoon:

  • Module 6: Acute Abdominal Pain: Developing a differential for RLQ pain (appendicitis), RUQ pain (cholecystitis), and diffuse pain (perforation, SBO, AAA).

  • Case-Based Session 2: The Diagnostic Puzzle. Cases focusing on AMS and abdominal pain, integrating imaging interpretation (CT head/abdomen).

  • Day 2 Recap: Don’t miss the aneurysm, the bleed, or the infarct.

Day 3: Shock, Sepsis, and Environmental Emergencies

• Morning:

  • Module 7: The Shock Patient: Differentiating hypovolemic, cardiogenic, obstructive, and distributive shock. Focus on sepsis recognition and early goal-directed therapy.

  • Module 8: Toxicology Overdoses: Management of common overdoses (opioids, acetaminophen, ASA, sympathomimetics).

• Afternoon:

  • Module 9: Environmental Emergencies: Hypothermia, heat stroke, anaphylaxis, and drowning.

  • Case-Based Session 3: The Resuscitation Bay. Managing simulated cases of septic shock, anaphylaxis, and opioid overdose.

  • Day 3 Recap: Finding the cause of collapse and reversing it.

Day 4: Infections, Trauma, and Special Populations

• Morning:

  • Module 10: The Febrile Patient: Workup of meningitis/encephalitis, sepsis in the immunocompromised, and necrotizing soft tissue infections.

  • Module 11: Basics of Trauma: Primary and secondary survey (ATLS principles), managing hemorrhagic shock.

• Afternoon:

  • Module 12: Pediatrics & Geriatrics: Unique presentations in special populations (e.g., pediatric fever, geriatric abdominal pain).

  • Case-Based Session 4: The Complex Case. Integrated cases involving trauma with medical problems and pediatric fever.

  • Day 4 Recap: One size does not fit all – adapting your approach.

Day 5: Integration, Procedures, and Mastery

• Morning:

  • Module 13: Common ED Procedures: Overview and indications for lumbar puncture, paracentesis, thoracentesis, and central line placement (theory and demo).

  • Module 14: Disposition & Documentation: Decision-making for admission vs. discharge. Writing a clear, concise ED note and mastering SBAR handover.

• Afternoon:

  • Capstone Activity: The Undifferentiated Patient: A series of complex, multi-system cases where participants must lead the assessment, create a differential, and initiate management.

  • Final Simulation & Mega-Code: A comprehensive final scenario incorporating a cardiac arrest/medical crisis.

  • Course Conclusion: Key clinical pearls, Q&A, and awarding of certificates.
